New Client Forms

All forms are now handled through a secure portal.
New clients will be required to:
1.
Create a username and login
2. Complete the intake form and sign the disclosure statement as directed.

Once you have created your login you will be redirected to change your password to something you remember. It's a good idea to do this right away because you will then have access to my secure server. If you need to send a secure email, or choose to select a video conference session, we will be able to "meet" via this portal. You will also be able to download any invoices/ receipts you may need for insurance reasons. Interacting through this portal is HIPAA compliant (meaning your electronic information is protected more than correspondence through my regular email or Skype).
You are welcome to continue to communicate with me via my regular email address as well.
